Strong Care Connect Membership Agreement
Effective Date: Upon payment of membership fees
Plan Administrator: Strong Care Connect, a legally recognized Discount Medical Plan Organization (DMPO)

  1. Membership Eligibility and Term
    Membership in Strong Care Connect (SCC) begins on the date your membership fee is received and successfully processed. Membership operates on a calendar-year basis (January through December). Patients enrolling between October and December will automatically receive an extended grace period, making their membership valid through the end of the following calendar year (approximately 13–15 months). From time to time, SCC may offer promotional extensions or renewal incentives at its sole discretion. A Family Membership includes the enrolling member, their spouse, and dependent children residing in the same household, as well as any full-time dependent students or other legally recognized family members living at the same address.
  2. Nature of the Plan
    Your SCC membership provides access to a network of participating healthcare providers who have agreed to offer discounted service rates in compliance with applicable state and federal law. Typical discounts may average 25% or more off a provider’s usual fee schedule, though the exact rate may vary by procedure and provider.
    This is not health insurance, nor is it a substitute for insurance coverage. SCC does not make payments to providers on your behalf and does not cover deductibles, co-pays, or any other insurance-related obligations. When using your SCC membership, you acknowledge that:
    • You are electing to use discounted self-pay pricing rather than billing a traditional insurance plan for those services.
    • No charges for covered services will be submitted to your insurance carrier while your SCC discount is active.
    • You may switch back to your insurance coverage at any time by providing written notice to your provider, but you may not use both simultaneously.
  3. Provider Relationship and Payment
    All payments for services are made directly between you and the participating provider.
    Payment is due at the time of service, unless otherwise agreed upon with your provider.
    Providers may charge a reasonable service fee if payment is not made at the agreed-upon time. SCC does not determine the medical necessity, quality, or appropriateness of care provided and assumes no responsibility for treatment outcomes.
  4. Use Restrictions
    Your SCC membership may not be used in conjunction with any third-party payer, including personal injury claims, worker’s compensation, or other insurance arrangements; or for services covered under an active insurance claim or settlement. If a third-party payer is used, your SCC membership will be temporarily suspended until that coverage or claim is no longer in effect. Memberships are not eligible for pro-rated refunds for periods of non-use or suspension.
  5. Fees, Renewals, and Refunds
    A member fee of $10 (USD) will be charged to the individual member and $15 (USD) per family. The family membership covers direct family members living in the same household. Family members living at a different address or non-family members living at the same address will not be covered by the same membership. All membership fees are non-refundable after 30 days from enrollment, except where otherwise required by law. Refunds requested within 30 days will be subject to a $5 processing fee. Renewal of your membership occurs on your first visit each calendar year to a participating SCC provider.
  6. Communication and Privacy
    By joining SCC, you authorize SCC to share basic membership and registration information with participating providers as necessary to administer your benefits. SCC fully complies with HIPAA and all applicable privacy regulations regarding your personal and health information.

    You may be required to present your SCC membership card or verify your membership status with SCC before receiving discounted rates.
  8. Limitation of Liability
    SCC does not credential or evaluate participating providers for quality, competence, or treatment type. SCC is not liable for the outcome of care, clinical decisions, or any disputes arising between you and your provider. Your participation in SCC simply grants you lawful access to a reduced fee schedule for healthcare services rendered by independent providers.
  9. Acknowledgment and Agreement
    By signing below, you confirm that:
    • You wish to enroll in the Strong Care Connect Discount Medical Plan.
    • You understand this membership is not insurance, but rather a contractual right to access discounted medical fees.
    • You agree to abide by all terms of this membership agreement.
    • Violation of these terms may result in suspension or termination of your membership.
